Guest Speaker Lecture Application Materials
Important Notes for Industry Expert Applications:
1. According to the Guidelines for Collaborative Teaching by the Center for Teaching and Learning Development (Click to View), applications for industry expert lectures must be approved by the departmental curriculum committee and submitted to the School Curriculum Committee for record before implementation and reimbursement.
2. Required documents for reimbursement (click to download forms):
- Lecture Application Form
- Industry Expert Evaluation Form
- Industry Expert Resume Form
- Expense Reimbursement Form
- Payment Receipt and Bankbook Copy of the Industry Expert
- Photos (at least two digital copies, upload to 114-1 Industry Expert Lecture Photo Upload Folder)
- Meeting minutes approved by the department or institute
File naming format for photos: IndustryLecture-(InstructorName)-(LectureDate)-Number (e.g., IndustryLecture-OOO-0903-01)
- Student Satisfaction Survey Form (Paper Version)
- Online Satisfaction Survey for Industry Expert Collaborative Teaching (Online Form)
- After the event, once students have completed the survey, please ensure that the response rate reaches at least 80%.
If instructors are concerned that students may not complete the online survey, a paper version may also be used; however, the reimbursement documentation must include the average score for each question and the response rate (number of respondents / total number of students enrolled).
